Care/Support Workers

Our care worker provides personal care and household tasks that is personalised for every individual we support through a tailored care plan. 

Main duties and responsibilities:
To provide personal and practical care for people with learning disability/challenging behaviour.

To assist the people, we support with their set activities of daily living like personal care.

To support those with decreased physical abilities and dementia.

To care for end of life.

To encourage the mental and physical activity of people we support to take part in their communities.

To support them with meals and menu planning.

To read and write daily records, take part in staff and people we care for meetings and in all mandatory training as directed.

To conform to all policies, procedures and guidelines laid down by both CQC and Prevail Support and Services in respect of carrying out these care duties and in other administrative aspects of our activities, as relevant.

To be available to work as and when agreed upon with Prevail Support and Services, giving 24 hours’ notice of any cancellation.

To carry out instructions from Managers and Supervisors, adhering exactly to the care plan of individual we support.

To actively listen to people we support, making considerations for their personal preferences always.

To ensure all home visits are for the time allowed, as scheduled and request the people we support signature on supplied time sheets at the end of each visit, showing accurate time of arrival and departure. Deliver time sheets weekly to Prevail Support and Services’ office for processing.

To maintain accurate, concise, up-to-date and timely records of the people we support, daily diary sheets, medication forms, financial transaction sheets and time sheets.
To document each visit in the daily house diary, specifying tasks undertaken, any relevant changes which have occurred and all other pertinent information. 

To write legibly the precise time of arrival and time of departure, always with accompanying date and signature.

To be more professional when making comments in the daily diary.

To work within Prevail Support and Services’ guidance regarding the handling of people we support finances, ensuring accurate documentation and receipting of any transactions.

To be familiar with Prevail Support and Services’ Health and Safety Policy and to promote safe working code of practices. 

To follow company policy by ensuring full compliance with infection control procedures.

To report and record any incident or accident which arises, no matter how small - whether to the people to support or the care worker.

To immediately report any noticeable changes in health, behaviour or circumstances of the people we support – whilst maintaining their right to privacy and confidentiality.

To contribute to review and update of the people we support care plans as needed.

To be aware of, and comply with, the tasks and activities which must NOT be undertaken as part of care duties, as detailed in Prevail Support and Services’ Policy on Prohibited Duties (Limits of Responsibility).

To advise Supervisors and/or Managers of any perceived problems or difficulties experienced with the service provided to the people we support.

To advise Supervisors and/or Managers of any ideas which might enhance or improve the level of service delivered to the people we support.

To participate in induction and foundation training and regular in-service training programmes as directed by Managers or Supervisors.

To make themselves available on a regular basis at an agreed upon time to review their personal and professional progress, which will be recorded in their personnel file, (this is available on request). This will be carried out as either a one-to-one or annual appraisal.

Health Care Assistant

Health care assistants connect on both a physical and psychological level with the people we support and their families. They undertake a range of activities under the supervision of a registered nurse and/or senior staff, having been assessed and deemed competent to perform the task at hand.

Main Duties and responsibilities:
Support the registered nurse in implementing the agreed upon plan of care for the people we support and in accordance with received instructions and training.

Help promote and maintain an environment that best meets the needs of the people we support, relatives and carers alike.

Contribute to a team approach to the people we support’ care in conjunction with all members of the multidisciplinary team and Nursing Residential environment.

Contribute to the legality and accuracy of the people we support care plan and/or other reports.

Participate in regular meetings and attend other meetings as requested.

Provide support and assistance to registered staff.

All team members are responsible for behaving in a manner that does not put themselves, people we support, visitors and other team members in danger or at risk in the clinical environment.
To maintain stock levels.

To assist in the maintenance of home cleanliness and tidiness and always adhere to infection control policy and procedures.

To ensure that the people we support can meet their dietary requirements.

To use a friendly, personal phone manner, responding to enquiries politely and ensuring all persons visiting the home are greeted and assisted in a welcoming and supportive manner.

To perform administrative duties as required to support registered nurses and clerical staff.

To escort people, we support to their health appointments.

Staff Nurse

Prevail Support and Services’ staff nurses work in GP surgeries as part of the primary healthcare team.

Duties of the job include:
Electrocardiograms (ECGs).

Complex or minor wound management.

Travel health advice and vaccinations.

Child immunisations and advice.

Women’s health (including cervical smears) & family planning.

Obtaining blood samples.

Smoking cessation.
Men’s health screening.

Sexual health services.

Providing medical attention, personal assistance, emotional support, or other care to co-workers or the people we support.

Staying current with advanced related technologies and applying new learnings where relevant.

Entering, recording, transcribing, storing, or maintaining information in written or electronic/magnetic form.

Developing specific goals and achievable goals, planning and organising.
